You start one session and put the phone between you. Tolk listens continuously, recognizes which language is being spoken, and speaks the translation out loud. No mic button, no switching languages, no passing the phone around.

How does Tolk know who is talking?

It hears both languages in the session and translates each side into the other automatically. You and the other person simply take turns like in any normal conversation.
